I have now gotten used to the hassle of manually enabling the add-in each time I open Outlook, but I would obviously prefer to get things operating properly. I have uninstalled iCloud, "repaired" Outlook, re-installed iCloud and the situation has not resolved itself. However, each time I closed Outlook and re-opened it, the add-in had once again been disabled and had to be manually enabled again. I enabled it via "file", "Options", "Add-ins" in Outlook and it then worked just fine. A few days ago, I noticed that the add-in was not enabled in Outlook. It has worked reliably for more than one year, reliably syncing data between my PC, "the cloud" and my iPhone 4s.

But, lo and behold, I blindly followed the below procedure and Outlook is working perfectly with iCloud. I am not sure WHY adding the plugin by browsing to the file (.dll) worked while simply letting the plug-in do it for you did NOT work.
